ANNOUNCEMENT OF INTENT TO CONTRACT FOR ARCHITECTURAL LED
PROGRAMMING, DESIGN AND ADMINISTRATIVE SERVICES
FOR THE
LEESAR REGIONAL SERVICE CENTER, FORT MYERS, FLORIDA

LeeSar, Inc. (“LeeSar”) is accepting requests for qualifications from architectural
consulting firms under the provisions of the Consultants’ Competitive Negotiation Act,
Section 287.055, Florida Statutes, relating to the regional service center to be developed
and constructed at 2727 Winkler Avenue, Fort Myers, Florida (“Facility Site”). The
services sought consist of all architectural, civil engineering, structural engineering,
mechanical engineering, and electrical engineering services needed for the programming,
design and contract administration of a regional service center to be developed and
constructed at the Facility Site. Firms qualified to submit for the project shall consist of
architectural firms licensed to engage in the practice of architecture under the provisions
of Section 481.219, Florida Statutes. All supporting design consultants will contract with
and work under the direction of the architect.

Qualified architects are invited to notify LeeSar of their interest in the project. A Request
for Qualifications and Statement of Qualifications and Performance Data Form (“Request
for Qualifications”) can be obtained from LeeSar by calling LeeSar’s administrative offices at 239-303-3446.

All interested architectural firms must complete the Request for Qualifications Form and
return the same to LeeSar’s administrative offices located at 5597 8th Street West, Lehigh
Acres, FL 339781 by January 11, 2010 at 3:30 p.m. in order to be considered for the
project. Any interested architectural firm not fully completing a Request for Qualifications
Form will not be considered for the project.

The Request for Qualification Form will be utilized to prequalify, select and short list no
fewer than three (3) architectural firms as the most qualified based on the following
criteria: ability of personnel, ability to meet time and budget requirements, previous
performance and references, recent experience, recent healthcare/industrial design
experience, geographical location of the firm, ability to respond to LeeSar in a timely
manner, demonstrated approach to similar projects and current and projected workloads.

LeeSar reserves the right to reject, in its sole discretion, any and all submittals.
